ATTENTION EMPLOYERS
The last date for receipt of the Return of Contribution under Regulation 26 of the ESI
Act, 1948 for the contribution period ended 30.9.2008 is 11.11.2008. Employers are
requested to file the duty filled in returns, with the designated Branch Office and also
to ensure that:-
1. Proper Proforma (New Form 5) as available in the ESI Act, is used.
2. Each and every column of Return of Contribution in Form-5, including
Insurance Numbers and Name of Dispensary, is correctly filled in;
3. Payment Challans are enclosed with the Return of Contributions, as
prescribed.
4. All employees who drew wages upto Rs. 10000/- during the period from
1.4.2008 to 30.9.2008 are included in the Return of Contributions.
5. A Self Declaration & A Self Declaration alongwith a Certificate from
Chartered Accountant is required to be given by the employers who have
employed 40 and more employees.
6. The employer has to furnish the detail of wages and contributions in respect
of disabled Person drawing wages up to Rs. 25000, w.e.f. 1.4.2008(vide
amendment in Rule 50 and 54 and insertion of new Rule 51 A in ESI Central,
Rules 1950) in separate sheet along with the return of contribution. In the
column against employer’s share of contribution in respect of such
employees, employer can write “To be paid by the Government.”
Submission of incomplete and defective Returns are liable to be returned
and are also liable to be treated as non-submission of Returns and the
same would attract the Penal provision as laid down under the ESI Act,
1948.
Employers who have not submitted From 01A(Annual Statement of Employees)
for the year 2007 so far, may please ensure submission of the same immediately.
Please ensure that the statement for the year 2008 is submitted before 31
st
January, 2009.
SUPER SPCIALITY TREATMENT CENTERS
The Director, E.S.I. Scheme has authorized Three Centers for the Super
Speciality Treatments as given below:-
1.Cardiac – U.N.Mehta Institute of Heart Diseases, Ahmedabad
2.Kidney – Institute of Kidney Diseases & Research Centre, Ahmedabad
3.Cancer – M.P.Shah Cancer Hospital, Ahmedabad.
More centres are being considered both for diagnostic & treatment.
REVOLVING FUND
Number of Beneficiaries who have availed Super Speciality Treatment and the
Expenditure incurred form the revolving fund during the financial years.
Amnesty Scheme
The Amnesty Scheme will be ending on 31.12.2008, Employers who
wants to avail the advantage may file the application before the closing
date.
