Format of Offer of Appointment order

MEMORANDUM
                  Sub:  Offer of appointment to the post of LDC in ESI Corporation.

Shri/Smt/Kum., is informed that he/she is offered a post of Lower Division Clerk 
in the Employees’ State Insurance Corporation on a temporary basis on the basis of the 
examination conducted by the ESI Corporation in December 2010. He/She is posted to 
2. The appointment is subject to the following terms and conditions:- 
  
a) He/She will be placed in the Pay Band – 1- Rs. 5200-20200 + 1900 (Grade
Pay) plus the usual allowances as admissible to the Central Government 
employees of similar rank at the same station of posting.
b) The appointment will be on a temporary basis for the present. He/She will 
be placed on probation for a period of two years from the date of joining, 
which can be extended without assigning any reason whatsoever. 
c) He/She will be governed by the Employees’ State Insurance Corporation 
(Staff and Conditions of Service) Regulations 1959 as amended from time 
to time. 
d) The appointment will be provisional and will be subject to production  of 
Original Certificates pertaining to his/her educational qualification/date of 
birth etc. and community certificate, if applicable, subject to the conditions 
that these certificates are in order and fulfill the eligibility conditions for 
appointment.  Further, on verification of his/her Character and Antecedents 
he/she should be found fit for service in the ESI Corporation. 
e) If at any later date after his/her appointment, his/her claim for SC/ST/OBC 
or any other claims pertaining to his/her eligibility for appointment is found 
to be false, his/her services will be terminated forthwith. (OBC certificate 
should be obtained on or after 20/10/2010)

f) No lien will be allowed to be retained in his/her parent office in case he/she 
is already in service. He/She will be governed by the Govt.  of India new 
contributory pension scheme effective from 1-1-2004. 
g) He/She is liable to be transferred and posted anywhere in India.  
h) He/She will not be entitled to any travelling allowance etc., for joining the 
post. 
i) The Corporation does not undertake to provide any residential 
accommodation to its employees. 
j) The appointment will be subject to the condition that he/she is medically 
examined and found fit for employment in the Corporation by a competent 
medical authority. 
k) The candidate, after submitting his/her acceptance and attestation forms 
shall simultaneously get himself/herself medically examined by the Male 
Civil surgeon of his/her Native District Head Quarters Hospital and forward the Medical Examination Report to the Additional Commissioner & 
Regional Director, Employees’ State Insurance Corporation, 143, Sterling 
Road, Nungambakkam, Chennai-34.  If he/she was examined earlier and 
declared fit for Government Service, the previous Government Authority’s 
letter indicating the position will be required to be furnished.  The fees for 
the Medical Examination will have to be paid by him to the Medical Officer 
and is reimbursable by the Corporation which will be subject to his/her
being declared medically fit. He/She will not be entitled to any travelling 
allowance for appearing for medical Examination. 
l) No candidate shall be eligible for appointment under the Corporation, if 
he/she has more than one living spouse.
m) Unless he/she is a citizen of India he/she has to produce a certificate of 
eligibility/affidavit as per the details given in the attached note.
n) He/She will be required to take an Oath/make an affirmative allegiance to 
the Constitution of India at the time of appointment.
o) The recruitment is against the vacancies on a Local/Regional basis.  Hence 
 he/she shall serve at least for seven years in Tamil Nadu. 
p) The following certificates in original should invariably be produced for 
verification at the time of joining along with the copy of acceptance. 
i. Eligibility certificate (Proforma attached) 
ii. Certificate to the effect that the candidate belongs to 
SC/ST/OBC/PH/Ex-Serviceman, where applicable. 
iii. No Objection Certificate from the parent department, if already employed in Government/Quasi Government Offices or Public 
Sector Undertaking/ Autonomous Body under the Central or 
any State Government. 
iv. Certificate of age/qualification and experience in Original.
v. Certificate showing the martial status in the Proforma attached. 
vi. Two passport size photos duly attested by a Gazetted Officer. 
vii. Medical Certificate of Fitness as mentioned at sub para (k) of 
para 2 above. 
viii. Transfer certificate from the institution were studied 
If he/she accepts offer of appointment made to him, he/she should forward 
his/her acceptance in Proforma A enclosed along with the attestation forms in 
Triplicate duly filled in, to the Additional Commissioner & Regional Director, 
(Administration Branch), Employees’ State Insurance Corporation, Panchdeep 
Bhavan, 143, sterling Road, Chennai-600 034 within 15 days of receipt of this offer. 
 Please note that in case no reply is received from you accepting the offer of 
appointment as aforesaid within 15 days this offer of appointment will be treated as 
cancelled. 
 He/She should report for duty at the place of posting as mentioned above on or 
before 31-5-2011.
